Sunnybrook Covid Vaccine Registration for eligible City of Toronto residents

Sunnybrook is accepting pre-registration for the COVID-19 vaccine from City of Toronto residents who meet the Government of Ontario’s eligibility criteria during the current phase of Ontario’s vaccine rollout.

Eligible City of Toronto residents who may pre-register for a COVID-19 vaccine during this phase of the Government of Ontario’s vaccine rollout include:

  • Any Toronto resident who is 70 years of age or older (those born in 1951 or earlier)
  • Indigenous adults
  • Adults who receive home care
  • Health-care workers (a person who works in a health-care setting in any capacity)

You must be a resident of Toronto to pre-register for a COVID-19 vaccine at Sunnybrook. If you meet the above criteria, pre-register for a COVID-19 vaccine by clicking the button below.

Please note: Pre-registration is for vaccine appointments to take place at Sunnybrook’s Bayview Campus. Vaccine appointments will be booked based on the government’s rollout plan, and pre-registering does not guarantee a vaccine appointment at Sunnybrook’s COVID-19 vaccine clinic.
